1/18/2024 EnchantaVaughn Approved QMHP-A began the session by inquiring about Chevelle’s current thoughts, feelings and emotions. QMHP-A discussed with Chevelle the importance of self-care and engaged Chevelle in a discussion surrounding her medication management. QMHP-A created a schedule with Chevelle to assist her with completing her ADL's and ILS's daily. The calendar consisted of projected times to complete certain activities of the day to include morning, noon, and night (i.e. cooking, hygiene, and cleaning). The calendar also provided Chevelle with information as it relates to washing clothes and how often she should be washing her clothes on a weekly basis. After discussion and review, QMHP-A taught Chevelle the difference between the ability to complete the task (physical and/or cognitive ability) versus the ability to recognize that the task needs to be done without prompting (cognitive ability). QMHP-A modeled each ability by engaging Chevelle in scenarios and how she is promoted to complete task and how she is not prompted to complete task within the home. QMHP-A engaged Chevelle in a skills building activity. QMHP-A explained to Chevelle that the purpose of this activity was to assist Chevelle with the importance of being able to separate and place her clothes in proper storage. QMHP-A demonstrated proper storing and separation of clothing through role play. Chevelle appeared to be receptive to QMHP-A’s interventions and services that were provided to her during the session. Chevelle said she takes care of herself and sometimes she does not, she is not going to lie. Chevelle said she brushes her teeth and shower, but she does skip days because there are days that her mind is telling her not to complete certain things or she just do not feel like it. Chevelle said she does separate her clothes but will wear the same because her others clothes are dirty. Chevelle will be lazy at times and will not look for other clothes. Chevelle indicated to QMHP-A that she just has so much going on in her life and in her mind personally that makes her depressed to the point where she does not want to do anything but lay around. During the role playing and demonstration, Chevelle discussed with QMHP-A that she always struggled in that area while also indicating that she was going to continue to implement learned techniques and stop feeling sorry for herself when it comes to certain things in her life that is not going the way that she wants them to go.
